Home Health is a program for those who need nursing care, rehabilitation, or therapy at home. The Home Health team of registered or licensed nurses, home care aides, and occupational, speech, or physical therapists work closely with your physician. For more information call (509) 574-3606.

The Home Health Team

Registered/licensed nurses provide direct, skilled nursing care according to the plan of care developed with the patient's physician.

Home care aides assist with personal care such as bathing, grooming and routine exercises.

TeleMed health monitoring combines technology and hands-on care. Registered and licensed caregivers daily monitor the patient's weight, blood pressure, pulse and oximeter through information sent via telephone lines. This allows the patient to stay at home and electronically send vital health information.

Occupational therapists instruct patients in techniques to relearn the tasks of daily living such as dressing, cooking, eating and bathing, as well as energy conservation and use of assistive devices.

Physical therapists assist with the patient's improvement of mobility, fall prevention and safety, ultimately reducing pain and helping the patient regain his or her maximum use of physical skills while recovering from an illness, injury or disability.

Speech therapists assist patients in improving communication challenges such as difficulty understanding other people, re-developing the ability to speak clearly or at all and literacy-related issues. The speech therapist may also help with difficulties in eating or swallowing.

Social workers assist with community referrals, caregiver issues and financial concerns. They can also assist with connecting patients to an emergency response using a system that provides patient safety and security via phone technology.

  • Skilled nursing and/or therapy services are required.
  • Patients are confined to a home environment.
  • Patient care is ordered by a physician.
  • Patients live within Yakima County.
  • The necessary care can be safely provided within a home environment.
